If you have booked a photography tour or ordered a product and chose to pay offline with a credit card authorization form, you can download the form below. If the form opens in your browser, please right click >>here<< and select Download or Save Linked File in your browser, to save it to your hard drive before filling it out.
You can fill out the form fields on the computer, but please print and sign the form before sending a scanned or photographed copy back to us.
IMPORTANT: If you do not forward a completed form within 48 hours of placing your order, your order will be cancelled. If for some reason you need more time than this, please let us know.
If you have not yet completed your order, please go to the tour or product page, and select “Pay offline with a Credit Card Authorization Form” as the payment option during checkout. This will ensure that we get the details we need to fulfill your order, and in the case of a tour booking, this is required to hold your space until payment is received.
